Initial Consultation - FREE
We meet one-on-one at an agreed location to discuss goals and intent for inquiring about obtaining a personal trainer. You talk, and I listen. You will be asked to complete a General Medical Questionnaire. This information provides me with programming information, and/or the need for a physician’s release to begin a physical training program. If you are working with a chiropractor, physician, physical therapist, or any other health professional that physical training could complement, you are asked to complete a HIPPAA release form allowing me to contact them to team together to provide you with the best results toward your personal goals.
Once your goals have been clearly defined and you feel comfortable moving forward, you will be provided the various training options and packages that are available to you. Training options and packages are outlined below. Active Daily Living aims to provide a package that works best for you, your time and your budget.

Single Training Session - $45
You may not feel comfortable making a commitment to a specific number of sessions and prefer paying as you. This package is for you.
Explore Package - $150
If you want to give personal training a fair shot, but not ready to commit - the Explore Package is for you. This is a one time only offer. It allows you to explore the services provided by Active Daily Living through four sessions and is a $7.50 per/session savings.
8 & 12 Session Packages - $320 | $450
These packages are designed to provide you monetary savings on your training. You save $5.00 per/session with the committed 8 training package and $7.50 with the committed 12 session package.
Small and Large Group - $60 | $80
This is a great way of making training more affordable. The cost of the session is split amongst all the participants. It is important that when forming a group that all persons have similar goals, so your training is focused on meeting your needs. Fitness level is less important since most exercises are easily modified to fit everyone’s current level. Group training can be fun, motivating and provide accountability. (Small Group: 2-3, Large Group: 4+)
Special Client Package - $400
If you are starting a training program post surgery, post physical therapy and/or other medical health reasons, the special client package may be for you. Active Daily Living would collaborate with your physician, physical therapist or other health professional to design programs that are safe and moving you toward an independently healthy life. You are asked to complete a HIPAA release form so Active Daily Living can speak with the appropriate persons in this collaborative effort to make you independent and healthy. After collaborating with your health professional, Active Daily Living will use this information to do an assessment and design programs specifically for you. This package includes ten training sessions.
Assessment Only - $50
You may simply want to see where your current fitness is at. Active Daily Living will assess balance, flexibility, and body fat.
6-month expiration on all packages, and 24-hour cancellation notice required.
